The scanner software that produces the images determines that. And I think there are many settings which influence that. There is not concrete definition of that origin point. It is defined so that other images which reference that same coordinate system can be defined relative to it (e.g. segmentation of tumor/organ, path of radiation beam etc).

While that is possible, it is frequently not the case.
The anatomical location of the image origin is a combination of how the patient is positioned on the table, how the imaging technologist sets up the scan, and implementation details of the scanner software. If reproducible image origin is important then you can create an imaging protocol document that describes details such as how to position the patient on the table, what anatomical reference point to align with the positioning lasers, and how to set various scanning parameters.
